These Terms & Conditions (“Terms”) govern your use of the website, forms, and communication services provided by Freedom Mini Dental Implants, PLLC (“we,” “us,” or “our”) for our dental practice in Louisville, Kentucky.

Use of Website and Services
Our website provides information about our practice and dental services. Using this website or submitting an inquiry does not, by itself, establish a doctor-patient relationship.

SMS Program and Message Types
The Freedom Mini Dental Implants, PLLC promotional text program sends messages about dental services, special offers, and invitations to schedule a free mini dental implant consultation to people who expressly opt in. Messages may be sent using automated technology.
After enrollment, you may receive a subscription confirmation. Related program administration messages may respond to HELP requests or confirm that you have opted out.
Consent Requirements
To enroll, enter your phone number and select the separate, optional marketing consent checkbox on our website consultation form. This checkbox is unchecked by default. You may submit the form without selecting it. Providing a phone number, submitting a form, or completing a quiz does not, by itself, enroll you in the promotional text program.
By selecting the marketing consent checkbox, you authorize promotional texts and calls from Freedom Mini Dental Implants, PLLC at the number provided about dental services, special offers, and free mini dental implant consultations, including automated texts and calls using prerecorded voices, as disclosed beside the checkbox.
